What is the role of your Case Manager in your home care? 

Why do you need case management?

According to CMSA (Case Management Society of Australia & New Zealand & Affiliates), whether in medical service or community services, case managers share the same goal – that is to assist individuals to access available and relevant resources necessary for them to attain their identified goals.

In the home care setting, case managers play an important role in helping the seniors to assess and identify their needs and goals, coordinate and leverage available resources (e.g. HCP) and liaise with relevant parties (e.g. care workers, 3rd-party service providers, government department, medical and allied health practitioners) in order to meet the home care needs and goals of the seniors.


The 4 main responsibilities of case managers in home care:

1. Ongoing Assessment 

Continuously assess the needs of clients, make and improve clients’ care plans

2. Coordination and Liaison

Arrange and manage services according to the care plan and clients’ needs and assist them to access other professional services (e.g.medical services)

3.Ongoing Monitoring

Through continuous communications, home visits, monitor the health and living conditions of the client;
Identify more needs of the client and provide professional advice and assistance;
At the same time, continue to pay attention to the usage of the HCP and provide recommendations and package upgrade assistance

4. Advocacy support and education

Advocacy support around navigating the aged care and health systems, including accessing aged care services if these are not already in place;

Educate and support the client, their carer and family members to better understand government policy and access resources and welfare that are available.

In a nutshell, Case Managers help address your various home care needs and ensure they can be met timely, properly and under the regulations and standards of the government.

Case managers help your specific health and well-being needs and goals to be fully noticed , and work together with you to maximise the value of your home care packages and enable you to live home independently and receive long-term appropriate care.
